 This directory contains 3,800 disciplinary actions taken by the Texas State Board of Medical Examiners against licensed physicians. It covers 20 years, from January 1, 1987 to June 1, 2007. The doctors are listed alphabetically by last name. Listings include information similar to that available in our Directory of Physicians and Surgeons, plus violation of the Medical Practice Act, disposition of action taken, date action was taken, and appeal status.

• A sample listing explains the separate parts of each doctor's six-line entry.
• Entries include primary and secondary specialties, full name, type of degree, sex, ethnicity, date and place of birth, medical school attended, graduation date, method/place and date of licensure, current license status, Texas license number, permit expiration date, mailing address, address of practice, violation of Medical Practice Act, date action was taken, disposition of action taken, and appeal status.
